In addition to providing a wide range of short-course training to care providers, we also continue to work very closely with our NHS colleagues, placing interim clinical and support staff within NHS Continuing Health Care (CHC) services.

We have worked within CHC since its original implementation in Norfolk in 2001 and have gained a wide range of expertise, knowledge and understanding in all areas:

• D2A Pathway (Discharge to Assess)

• Clinical Assessment and Review (checklist and full DSTs)

• Brokerage/fast-track

• Acute Hospital CHC Discharge team

• Retrospective/PUPOC Assessment

• Project Work
